HTTP/2 Ping Abuse Policy (transport/ping_abuse_policy.cc)

Implements gRPC chttp2 transport policy to detect and limit abusive ping rates from peers. Reuse for HTTP/2 connection abuse mitigation and configurable ping interval enforcement.
